Slide comes off after dry fire

Two things I’m trying to work out after looking through a number of posts and a fair bit of googling:

• the ejector bumps the back plate ever so slightly. All areas to be clear of material so there seems to be no visible reason for not aligning. No evidence of parts bending either.

-bigger problem is that the slide comes right off after a a dry fire, implying that recoil from will produce a similar and higher stakes result. I’ve provided video to show the state of things. I’ve disassembled and reassembled half a dozen times to see if I missed a step or have something out of wack. Drawing a blank.

Any help is appreciated.
